IM epinephrine in the thigh, now — everything else is adjunctive.
Recognition and treatment of anaphylaxis in the ED: intramuscular epinephrine as first-line therapy, aggressive fluid resuscitation for vasodilatory shock, epinephrine infusion for refractory cases, airway planning, and observation with an autoinjector discharge plan.
